What urine culture actually shows
The test answers three critical questions:
- Microbial growth & load: Quantified in CFU/ml, with a diagnostic threshold usually ≥10⁵ CFU/ml on a midstream sample. Values <10⁴ are often considered contamination or borderline, drug-sensitive infections.
- Pathogen identification: Gram-negative (E. coli, Klebsiella, Pseudomonas) or Gram-positive (Enterococcus, Staphylococcus saprophyticus).
- Antibiogram: Sensitivity/resistance across more than 10 antibiotic classes.
Culture is invaluable for distinguishing a genuine infection from sample contamination, particularly in ambiguous or recurrent cases.
When it is requested and why it is critical in men
Main indications:
- symptomatic urinary tract infection
- a febrile urological episode
- recurrent infections
- failure of empirical antibiotic therapy
- pre-operative screening before an endourological procedure
In a man with a urinary infection, culture is usually accompanied by an assessment for obstruction, post-void residual, stones, or prostatic involvement.
Likely contamination
Multiple organisms at low counts, inconsistent with the clinical picture.
Borderline result
Needs re-evaluation alongside symptoms, urinalysis findings, and how the sample was collected.
Clinically significant infection
Clear growth of a pathogen with matching symptoms and documented inflammatory activity.
Correct sample collection and common mistakes
For a reliable result:
- a midstream urine sample after local cleaning
- a sterile container
- prompt delivery to the laboratory
- avoiding prolonged storage at room temperature
Common mistakes (poor hygiene, an unsuitable container, or excessive delay) lead to false-positive or false-negative results.
